Now Registering Students for the 2026/2027 School Year

We are now registering students for the 2026-27 school year. In accordance with an agreement between the school districts of Humboldt County, December and January are the only months for students to register for a school outside of their district of residence. All interested families should contact Garfield School regarding potential enrollment for any class, TK- 6th grade. Parents of in-district Transitional Kindergartners and Kindergartners should register now as well, helping us to determine the number of additional spots that are available for out-of-district students in each grade.

Forms – MANDATORY FORMS

INSTRUCTIONS:
The Back-to-School Forms Packet contains all of the mandatory forms required by Garfield School. (TK/K students will also need to download, print, fill out, and return the forms in the TK/K Records & Forms section below.) You may choose to fill out these forms and email them to kblair@garfieldschool.org, or you may fill them out, print them, and return them to the school office. Please contact the school office at (707) 442-5471 if you have any questions.

Please note that the mandatory forms in this packet require parent and/or guardian signatures. Typing your name in the designated signature field(s) on each form acts as your formal legal signature and confirms your explicit agreement to all terms and conditions stated therein.

Forms – FOR NEW TK/KINDERGARTEN STUDENTS

INSTRUCTIONS:
The first two forms listed will need to printed out and filled out by healthcare professionals. The Student Transition Form may be filled out and emailed to kblair@garfieldschool.org, or you may fill it out, print it, and return it to the school office along with the other two required forms listed.

Please note that all new TK/Kindergarten students also need to provide their complete vaccination records. Please call your child’s healthcare provider or pediatrician and request a copy if you do not already have this.

Oral Health Assessment (Print and have your child’s dentist fill out)

Below you will find some optional forms. These are not required, but if applicable, may be filled out and emailed or dropped off at the school office.

Volunteer Registration & Survey (Live Scan and TB Test required unless otherwise stated)
Field Trip by Private Vehicle Driver form (Please include proof of coverage at minimum requirement)
Booster Club Sign-up Form
Consent for Medication at School (Must be filled out by your doctor)

Please remember that if you are hoping for your child to transfer to Garfield School District from a different district, the timeframe to fill out and submit the inter-district transfer agreement is December 1st through February 1st. If you are unsure what district you currently reside in, you may use the District Locator tool found on the Humboldt County Office of Education’s website.
